Leavers and exit process
UHB is committed to retaining staff and ensuring a great working environment for everyone. However, we recognise that at times and for a variety of reasons, staff will leave their role or the Trust.
A well-managed leaving process helps to ensure that the transition is as smooth and stress-free as possible for all parties.

This form is to be used if you are leaving your role to move to another within UHB, if you are leaving your role to move to a UHB+ (Bank) only role or if you are leaving your role and UHB entirely. Once this form is completed the leavers process will begin for whichever option you have selected.
